Customizer Theme Switcher: Fix preview URL for subfolder installs.

Use `$_SERVER['HTTP_HOST']` and `$_SERVER['REQUEST_URI']` instead of `site_url()` and `$_SERVER['REQUEST_URI']`.

fixes #31896.
