Add GitHub issue templates 
Credit to @Andre601 and PR #1735 for the original proposal of these templates.

I've held off from adding this for a while, but I now feel that the GitHub UI is nice enough to justify the change.

Reproduction steps

  1. Open the config.yml file and set example to true.
  2. Restart the server
  3. Run /lp user example permission set x from the console
  4. See error

  • Server type/version: ExampleSpigot running version 1.12.2 build ???
  • LuckPerms version: v???
